the U.S. has no structure in place to resolve such issues. Who should be responsible for protecting the public from unethical, ineffective, or unduly hazardous procedures and how should it be done? Can effective oversight activities be accomplished without destroying confidence in the health professions and institutions?
Are you talking about health care people doing hazardous, unethical things? You really need to be more specific.
But the US, well the states have laws regarding the behavior of medical professionals and if they are doing unethical and hazardous procedures, report them to the state licensing agency. There is already oversight in place. In addition to the state licensing agencies, hospitals have their own ethics and research committees.

Those are already in place and working. The past few generations, including mine, are so spoiled and sheltered here we expect to be protected from every risk. That insanity has become so bad we are squandering a lot of our prosperity trying to squeeze the impossible last bit of risk out of our lives.
Greedy and unprincipled people are now suing for every little mistake or oversight and too often actually are awarded damages. The whole spoiled and sheltered society is ridiculously pampered now. The unprecedented great advantages of modern life are so common to them they consider it, and more, to be their right.
